Advance Care Planning

Who would help make your health care decisions if you become too ill to voice your wishes yourself? What kind of services and measures would you like to have in place? Advance care planning means talking with your family and healthcare providers about the kind of care you want if you become unable to make those decisions for yourself, and let’s everyone know what your wishes are. Our team can assist in with advance care planning by answering your questions and providing information and assistance with forms. If you need to update your current information, or need assistance with advance care planning let us know and we will set up a visit to discuss.

Transitional Care Management

he purpose of Transitional Care Management is to  provide a smooth transition when a person is discharged from a hospital, rehabilitation facility, or other in-patient setting. We will make a face-to-face within days of the patient returning home to meet with the patient and caregiver(s) to discuss care, educate on medication and treatment, and determine if other services might be needed to assist with the care and recovery of the patient. We will provide support and offer resources to help support the patient and family, and communicate with any community services already in place.

Once an initial visit is made, additional care will be given telephonically and through additional visits to continue to address the needs of the patient and coordinate care between any agencies providing the patient with services at home.

Parker At Your Home services are available in Brooklyn, Queens, Nassau County and parts of Suffolk County.
