Call 718-289-2606 if you have any questions or would like more information; otherwise, you can email completed paperwork (provide PDFs here as well) as well as a copy (front and back) of your insurance cards to PAYD@parkerinstitute.org

Once we have your completed paperwork we will schedule a nurse practitioner to make a visit within the next two weeks.

We are available 24 hours a day, seven days a week. If the office is closed there is always someone on call to handle urgent concerns.

Parker at Your Door will become your primary physician; however, if you are seeing a specialist for another condition (urologist, cardiologist, neurologist, etc.), you can still see that provider. With your permission, we can share any notes and diagnostic test results with your other physicians.

We will provide any information the hospital needs at your request. If we have a practitioner at the hospital they can follow up as well. Once discharged, we will provide transitional care management to ensure that your discharge goes smoothly.
