The Guiding an Improved Dementia Experience (GUIDE) Model

Compassionate Support for Individuals Living with Dementia and Their Caregivers

The Guiding an Improved Dementia Experience (GUIDE) Model is an initiative developed by the Centers for Medicare & Medicaid Services (CMS) to enhance the quality of life for people living with dementia and to support their caregivers. The program helps individuals remain safely and comfortably in their homes while reducing caregiver stress through coordinated, person-centered care.

Parker At Your Door, a trusted community health program of the Parker Jewish Institute for Health Care and Rehabilitation, proudly provides the full range of services covered under the GUIDE Model, beginning July 2025. Through this program, families receive expert guidance, compassionate support, and access to valuable community resources.

By integrating expert clinical care, education, and community-based resources, the GUIDE Model empowers families to navigate the complexities of dementia with confidence. Participants receive the support they need while continuing to cherish meaningful moments together at home.

As part of its ongoing commitment to excellence in community-based care, Parker At Your Door is honored to participate in the CMS GUIDE Model. This initiative reflects Parker’s dedication to supporting individuals living with dementia and the caregivers who care for them.

Eligible GUIDE beneficiaries
Medicare and dually eligible beneficiaries who have an attested dementia diagnosis by a participating clinician are eligible if they meet additional criteria, including:

  • Not receiving hospice services
  • Living at home or in a community-dwelling
  • Enrolled in Medicare Parts A & B or dually eligible for Medicare and Medicaid
  • Not enrolled in Medicare Advantage or Special Needs Plans (SNPs)
  • Not residing in a long-term nursing home
  • Not receiving PACE services
Personalized Care Coordination

A dedicated Care Navigator will help you access resources, coordinate services, and develop a care plan tailored to your specific needs, ensuring you get the best support for your specific situation.




Caregiver Respite


Through GUIDE, participants gain access to respite services, which are temporary services provided to a beneficiary in their home, at an adult day center, or at a facility that can provide 24-hour care for the purpose of giving the unpaid caregiver temporary breaks from their caregiving responsibilities.

Family Caregiver Training & Support

Family caregivers are equipped with the skills and resources needed to care for their loved ones with dementia. Education and support programs empower you to overcome challenges, relieve stress, and build confidence.




24/7 Dementia GuideLine
